4. A statement that you have a good faith belief that use of the material is not authorized
5. A statement, under penalty of perjury, that the information is accurate and you are the copyright owner or authorized to act
6. Your physical or electronic signature

Counter-Notification

If you believe your content was wrongfully removed, you may submit a counter-notification pursuant to 17 U.S.C. § 512(g). Upon receipt of a valid counter-notification, we will restore the removed material within 10–14 business days unless the copyright owner files a court action.
